Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a shared responsibility. The requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Clinical Coordinator or Social Work Manager, the Social Worker provides assessments and services to meet the emotional and social needs of residents and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, the Social Worker offers psychosocial care, system navigation, and advocacy. The role involves developing social work services to maintain or enhance residents' health, social well-being, and functioning.

Qualifications:

  • Bachelor's Degree in Social Work from an accredited university
  • Registered and in good standing with the British Columbia College of Social Workers
  • Two years' experience in a healthcare setting, preferably in Gerontology or Chronic Diseases
  • Knowledge of healthcare legislation, community resources, and programs

An equivalent combination of training and experience may be considered.

Skills and Abilities:

  • Assessment and Treatment: Ability to complete client assessments and implement appropriate interventions
  • Teaching: Capable of instructing clients and groups
  • Knowledge Integration: Use current research to inform practice
  • Communication: Effective verbal and written communication skills, including conflict resolution
  • Critical Thinking: Data evaluation and problem-solving skills
  • Human Caring and Relationship-Centered Practice: Promote client-focused care, cultural sensitivity, advocacy, and social justice
  • Management: Organizational skills, prioritization, teamwork, and collaboration
  • Leadership: Ability to plan for change and professional development
  • Teamwork: Foster collaboration and excellence in care
  • Equipment: Proficiency in computer skills, including Windows, email, and word processing; operation of related equipment
  • Physical ability to perform job duties
