The Windsor Regional Hospital social worker will assist patients from admission through discharge with their psychosocial needs, including assessment, treatment and community reintegration. Working closely with a multidisciplinary team, the role focuses on quality, confidentiality and patient-centered care while linking patients to community services and supports.
Minimum 1–2 years discharge planning experience in Acute Care is preferred, with a Bachelor’s in Social Work and active registration;
The Medicine / Surgery and Women & Children's Social worker will assist Windsor Regional Hospital consumers, from admission through discharge, with their psychosocial needs. This care includes specialized assessment, treatment, rehabilitation and community reintegration. This position will improve care of the patients in the hospital and establish linkages with community partners which support and/or care for the patients. In addition this position provides for a unique collaborative relationship with the multidisciplinary team. The Social Worker will provide comprehensive assessment, rehabilitation and treatment to the patients and their families. The Social Worker complies with the Occupational Health and Safety Standards for the hospital and is proactive in promoting a safe work environment. The incumbent is responsible for delivery of care consistent with the goals and objectives of the hospital and the standards of relevant professional organizations. This position contributes to the hospital mission by being a team member who ensures confidentiality, quality, and client-centered health care.
