Humber River Health is seeking a Social Worker to join the Wilson Site team on a part-time basis. You will perform psychosocial assessments, crisis intervention, discharge planning, and family counselling, coordinating with patients, families and the health care team.
The role supports patient flow across acute, RCCs, and community settings, requires MSW and current professional membership, strong communication and organizational skills, and the ability to work independently or with a team.
