CarePartners is seeking a Social Worker to join our Rehabilitation Team in North Wellington. The role focuses on professional assessments, care planning, and collaborative goal-setting with the care team to optimize patient outcomes in home and community settings.

Responsibilities

  • Conduct professional assessments of patients in the community and in homes.
  • Develop and maintain effective Care Plans with respect to Community Health Services.
  • Set time-specific, measurable goals in collaboration with the care team.
  • Work independently and as part of an interdisciplinary team.
  • Provide assessment, intervention and discharge planning within scope of practice.
  • Be knowledgeable about Community Resources and funding options.
  • Guide workplace Health and Safety practices.

  • This position offers a competitive compensation grid, with progression based on hours worked and role‑specific criteria. The Social Worker per visit range is $74.48 to $86.85 and the hourly range is $37.23 to $43.41. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
What The Role Involves
  • Professional assessment of patients in the community and in their homes
  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome
  • Working with the patient on appropriate time‑specific, measurable goals and outcomes in collaboration with the care team
  • Working independently and as an integral member of an interdisciplinary care team
  • Provide assessment, intervention and discharge planning, within the scope of practice
  • Demonstrates a clear understanding of Community Resources, including funding
  • Providing active guidance for workplace Health and Safety
  • Performing other duties as required
What You Bring
  • Possess Master’s Degree (or equivalent) degree in Social Work from a recognized university
  • Current, unrestricted license to practice with Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Proven computer skills, including email, word processing, work scheduling systems
  • Strong assessment skills and the ability to participate in effective treatment programs
  • Provide one‑on‑one therapeutic support with clients
  • Promotes independence of the client in the home and in the community
  • A reliable vehicle for some regular travel
  • Excellent English verbal and written communication skills
  • Clear Background and Vulnerable Sector Check
