Job summary

Spectrum Health Care is seeking a Social Worker to join a dedicated team delivering integrated community care in the Etobicoke area, extending to Mississauga. The organization has over 40 years of experience and is recognized among Canada’s Best Managed companies.

Compensation is based on experience with hourly pay from $60 - $85 plus eligible travel pay. A strong focus on home-based care and collaboration with a multidisciplinary team defines this role.

Qualifications

• Completion of a Bachelor of Social Work from an accredited university.
• Master of Social Work preferred.
• Registered with the Ontario College of Social Workers & Social Service Workers.
• Minimum 5 years of experience working as a Social Worker.
• Experience in a community setting is an asset.
• Valid Ontario driver’s license and a reliable vehicle.
• Knowledge of diverse patient populations including frail seniors and multiple comorbidities.
• Proficiency with Microsoft Office and virtual communication platforms.

Responsibilities

• Engage in meaningful dialogue with the patient to assess needs and challenges across physical, emotional, psychological and social factors.
• Develop a tailored care plan using a client-centered approach.
• Set measurable goals to track progress and outcomes.
• Coordinate care through collaboration with a multidisciplinary team.
• Provide ongoing support, guidance and encouragement to patients.
• Assist with identification, referrals and navigation of community resources.
• Conduct regular follow-up visits (in person or virtual) and adjust the plan as required.

Job Description

• Engage in meaningful dialogue with the patient to gather a comprehensive assessment of the client's needs and challenges, including physical, emotional, psychological and social factors.
• Develop a tailored care plan to address the client's specific needs and goals, using a client-centered approach.
• Set measurable goals to provide clarity, track progress, and ensure that interventions and services are targeted towards specific outcomes.
• Ensure a holistic and coordinated approach to the patient's care plan through collaboration with a multidisciplinary team.
• Provide ongoing support to the patient, offering guidance and encouragement to help them navigate challenges and achieve their goals.
• Provide assistance with identification, referrals and navigation of appropriate community resources to help patients reach their target goals.
• Conduct regular follow-up visits (in person or virtual) to assess the client's progress, addressing any emerging issues and adjusting the care plan as required.
