Job summary

SE Health is seeking a Social Worker to support a roster of clients in the Mississauga/Halton area within a hybrid work model. You will conduct assessments, set goals, and implement client-centered treatment plans to help clients progress in their home and community settings.

Eligible candidates hold an MSW and are in good standing with the Ontario College of Social Workers and Social Service Workers, with preferred experience in acute/rehab or community rehab settings.

Qualifications

  • MSW degree and active Ontario SW registration.
  • Experience in acute/rehab or community rehab preferred.
  • Flexible, self-directed professional, able to work effectively and collaboratively with an interdisciplinary rehab team.
  • Knowledge of community resources is an asset.
  • Strong interpersonal, organizational and communication skills.
  • Positive work and attendance record is essential.
  • Able to adapt to new computer applications.
  • Hybrid work approach: remote and client-site duties.

Requirements
  • A graduate of an accredited program in Master's of Social Work and are a member in good standing of Ontario college of Social Workers and Social Service Workers
  • Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred
  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team
  • Sound knowledge of community resources is an asset
  • Effective interpersonal, organizational and communication skills
  • A positive work and attendance record is essential
  • Ability to adapt to new computer applications
  • This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting
