Position Summary

Reporting to Administrator, the incumbent will be responsible for relating to the residents and their families in the facility. He/she will perform the role of counselor and case manager. He/she will assist in conducting psychosocial assessments, counseling, coordinating admissions and discharges, and organizing support groups and educational programs for residents and families. Working with the Activation Supervisor, the incumbent will also develop and support volunteer and pastoral services.

Main Duties

  • Establish policies, procedures, and objectives for the Social Services Department.
  • Complete social, emotional assessments and conduct tests for cognition of all residents.
  • Interview applicants, residents, and their relatives to obtain social histories.
  • Assist in creating suitable physical, social, and spiritual environments for new and current residents.
  • Assist residents in securing applications and equipment as required, such as orthopedic, hearing, dental or optical aides, or wheelchairs.
  • Provide advocacy where the resident/family is unable to do so in the role of a client representative.
  • Maintain good relations between Home, residents, relatives and the community and provide counseling services and crisis intervention.
  • Collect and provide residents and Home’s staff with information about community health and welfare resources.
  • Work closely with the Director of Resident Care to provide support to staff, residents, and family regarding Palliative Care.
  • Coordinate liaisons with Community Care Access Centre placement co-ordination services.
  • Support the Activation Supervisor to plan programs and activities for residents.
  • Actively participate in Interdisciplinary Committee meetings and take follow-up actions as required.
  • Coordinate and chair the Ethics Committee meetings.
  • Assist the Resident Council and Family Council.
  • Participate in the recruitment, orientation, training, supervision and recognition of volunteers.
  • Act as support in the planning, developing, and managing of community services and outreach programs.
  • Investigate and report complaints or concerns from residents/families.
  • Prepare reports, surveys, and monthly audits for Social Services.
  • Other duties as assigned from time to time.

Qualifications

  • Degree in Social Work and member of the Ontario Association of Professional Social Workers.
  • Knowledge of gerontology, experience in a Long-Term Care setting preferred.
  • Excellent interpersonal, communication and organizational skills.
  • Demonstrated family, group, and crisis intervention counseling skills.
  • Strong consultation skills and flexible to respond to the needs of others.
  • Ability to work independently without supervision and as a member of an interdisciplinary team.
  • Able to work flexible hours including evenings and weekends.
  • Proficiency in both English and Chinese.
