Job summary

Region of Waterloo's Sunnyside Home seeks a Coordinator, Community Client Supports (Seniors) to provide biopsychosocial client support, assess eligibility for homemaking and supported living, and collaborate with care partners and providers. The role involves complex coordination, advocacy, and ensuring regulatory compliance in a person-centered framework.

The incumbent will lead service planning, monitor eligibility, and work with contracted agencies to ensure client needs are met.

Qualifications

• Post-secondary degree or diploma in social services/health-related discipline and related experience.
• Knowledge of health resources, assessment, referral, supportive counselling, and crisis intervention.
• Ability to work with older adults, care partners, and diverse service providers.
• Understanding of policies, legislation, and service delivery in community settings.

Responsibilities

• Provide consultation and supportive counselling to clients, care partners, and health care teams.
• Oversee admissions support, assess eligibility and suitability for supported living and alternative housing.
• Respond to information requests, screen referrals, monitor waitlists, and coordinate transitions.
• Coordinate services with contracted agencies, community partners, and internal teams.
• Develop and monitor individualized service plans with clients and partners.
• Promote resources, make referrals to community services, and advocate for clients.
