Job summary

S&R Group is seeking a part-time BSO Lead Registered Practical Nurse (RPN) to join the Clinical department and support residents in long-term care. You will apply your dementia care expertise, RAI documentation knowledge, and advocacy for residents’ rights in a collaborative team environment.

Responsibilities include comprehensive assessments, care planning, non-pharmacological and pharmacological management, quality improvement, and coaching peers to ensure high‑quality care and timely

Qualifications

• Extensive knowledge in dementia care and related conditions.
• Experience with RAI assessments, RAPs and care planning.
• Advocates for residents' rights and person-centered care.
• Strong clinical-reasoning and assessment skills.

Responsibilities

• Provide care for residents with dementia, delirium, mental health, and behavioural changes.
• Document and update RAI assessments and care plans.
• Collaborate with care teams to ensure safe, high‑quality care.
• Coach and mentor team members; participate in quality improvement.

Job description

BSO Lead Registered Practical Nurse (RPN)

• Part-time
• Department: Clinical
• Compensation: up to CAD 30.38 - hourly

• Demonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, other neurological conditions, and those who experience an acute behavioural change
• Experience/ familiarity with RAI documentation including assessments, Resident Assessment Protocols (RAPS) and care planning functions
• Belief in the protection of individual’s rights and advocacy role for the resident/clients
• Demonstrated excellent clinical-reasoning and critical thinking skills to ensure preventative and early detection skills; strong assessment, intervention and care planning skills
• Ability to deliver non-pharmacological and pharmacological management approaches (pharmacology skills)
• Demonstrated trouble‑shooting and problem‑solving approach relative to the situation within the scope of practice
• Demonstrated commitment to seeking and sharing practice‑based experiences in two‑way exchange
• Provides facilitation, coaching, mentoring and adult education skills
• Ability to adapt positively and productively to changes in the work environment
• Ability to identify indicators of compassion fatigue and plan effectively for mitigation
• Utilize an appropriate problem solving approach relative to the situation within their scope of practice
• Demonstrated effectiveness as a team member through collaboration, respect and effective communication skills in interacting with clients, family members and other members of inter‑professional care teams,(clients, family members move to front) including community and primary care team members
• Experience with quality improvement processes an asset
• Experience in GPA Coaching

• Prior experience with clients in long‑term care and the community
• Two (2) years current experience as a Registered Nurse in long‑term care is desired
• Experience running the BSO Program
