Job summary

Province of Nova Scotia Regional Centres for Education is seeking a SchoolsPlus Facilitator to manage a complex caseload of students and families, coordinating school and community services to support well-being and academic success.

You will develop and implement comprehensive service plans, liaise with partners, and oversee outreach programs within the Chignecto Family of Schools hub, Cumberland County. Strong social work background and collaborative skills are essential.

Qualifications

• Master of Social Work or Bachelor of Social Work with related experience.
• Experience in integrated service delivery to children, youth and families with complex needs.
• Current registration with NS.Social Workers or eligible to register.
• Familiarity with CCRCE regions and allied health, justice, health services.
• Ability to develop budgets and supervise funding usage for resources.
• Maintain records, confidentiality and create case plans.
• Ability to work independently and collaboratively with multiple service providers.
• Experience in group facilitation and advisory committees.
• Engage with diverse communities within a multi-disciplinary team.
• Excellent interpersonal and written communication skills.
• Able to manage a demanding caseload with minimal supervision.
• Willing to work flexible hours and over the summer.
• Valid driver’s license and willingness to travel.

Responsibilities

• Manage a complex caseload of students referred within the hub of schools.
• Liaise between school and community to coordinate and expand services.
• Develop and monitor comprehensive service plans for students and families.
• Oversee and implement programming to support student well-being using multi-tiered approaches.
• Complete tri-annual reports for internal and departmental evaluation.
• Consult with students, families, school staff and community partners to assess needs.
• Coordinate links to services and assist with problem solving for presenting needs.
• Liaise with regional Centre staff and external agencies to improve access to services.
• Oversee programs supporting SchoolsPlus model and supervise outreach workers.
• Summarize activities and outcomes for reporting and planning.
