Job summary

Chignecto-Central Regional Centre for Education (CCRCE) seeks a SchoolsPlus Facilitator to manage a complex caseload and act as liaison between school and community, coordinating services for children, youth and families. The role supports a multi-agency approach within a hub of schools, focusing on prevention and integrated service delivery.

The successful candidate will have a Masters in Social Work (or Bachelor’s with additional experience), be registered or eligible with NS Social Workers,

Qualifications

• Masters of Social Work plus two years related experience; or Bachelor of Social Work plus four years related experience.
• Experience in integrated service delivery and direct service delivery to children, youth and families with complex needs.
• Current registration with the Nova Scotia College of Social Workers, or eligible to become registered.
• Familiarity with CCRCE regions and community resources.
• Ability to develop budgets and supervise funding utilization.

SchoolsPlusis a collaborative interagency approachsupporting the whole child and family. The vision for SchoolsPlus is for schools to become centres of service delivery, enabling enhanced collaboration by bringing professionals and programs together to support children, youth, andtheir families. SchoolsPlus promotes an integrated service delivery model through co-location and/or partnership of services within a school such as: Health, Justice, Opportunities and Social Development and other community organizations. SchoolsPlusworks tosupportpreventative, proactive interventions for students and families.

Duties:

Under the supervision of the SchoolsPlus Lead, the SchoolsPlus Facilitator manages a complex caseload comprised of students referred within their designated hub of schools; acts as a liaison between school and community to advocate, coordinate, and expand services for students and families; develops and implements comprehensive service plans for students/their families with complex needs and then monitors and assesses their progress; and provides/oversees programming and interventions to support student well-being using a multi-tiered system. The SchoolsPlus Facilitator completes tri-annual reports documenting the work within their hub and other pertinent statistics for the purpose of both internal office and departmental evaluation and planning. They consult regularly with students and their families, as well as school-based staff, and community partners to determine the ongoing needs of students and their families and plans accordingly based on their social work knowledge, skills and values. They monitor, assess, and evaluate progress by maintaining ongoing contact with students and families who are on the SchoolsPlus caseload as well as service providers connected with the student. They coordinate and link students and families to support and services as available. They assist student/families with problem solving for social, emotional, academic, behavioral, legal, and other identified presenting needs. The SchoolsPlus Facilitator liaises with regional centre personnel and outside agencies to increase access to services for identified students and their families. The SchoolsPlus Facilitator, oversees and at time facilitates programs and activities that support the SchoolsPlus model to achieve student success, support student well-being, and enhance student engagement while developing and maintaining a positive public image and effective community relations. They will be responsible for the direct supervision of the SchoolsPlus Community Outreach Workers and Child and Youth Care Practitioners within their respective area.

Qualifications:

• Masters of Social Work plus two years related experience; or Bachelor of Social Work plus four years related experience.
• Experience in integrated service delivery and direct service delivery to children, youth and families with complex needs.
• Current registration with the Nova Scotia College of Social Workers, or eligible to become registered.
• Familiarity and knowledge of the Regional Centre, Opportunities and Social Development, Justice, Health, and community resources
• Capability to develop budgets and supervise funding utilization to ensure efficient and effective allocation of resources.
• Ability to keep accurate records, maintain confidentiality and create case plans.
• Experience working independently as well an ability to work collaboratively with various service providers.
• Demonstrated experience in group facilitation and working with Advisory Committees.
• Ability to engage and work with diverse communities and as part of a multi-disciplinary team.
• Excellent interpersonal, oral and written communication skills.
• Ability to plan and organize a demanding caseload with minimum supervision.
• Willing to work flexible hours and over the summer.
• Valid driver’s license and willingness to travel as needed.
