Kingston Community Health Centres seeks a Mental Health Counsellor to join the Rural Frontenac, Lennox and Addington Allied Health Program. The role delivers individual and group counseling, case management and collaboration with physicians and stakeholders across primary care in the RFLA region.

Qualifications

• Master of Social Work with psychotherapy experience.
• Registered with Ontario College of Social Workers and Social Service Workers or College of Occupational Therapists of Ontario.
• Minimum of three years recent experience in health/social services and community-based delivery.
• Experience with brief solution-focused counselling and single-session therapy.

Responsibilities

• Complete social histories and assessments to identify social/emotional needs; provide counseling and interventions.
• Refer and advocate to community resources to connect clients with services.
• Liaise with social/health services regarding the RFL&A AHP and role of the MH Counsellor.
• Provide psychosocial consultations to the care circle on client functioning.
• Provide emergency/crisis support and referrals as needed.
• Maintain case records in electronic health records and support quality improvement.
• Participate in chart reviews, case conferences and team meetings.
