Job summary

Hamilton Family Health Team is seeking a Mental Health Counsellor to deliver a broad range of services in a family physician's office. You will assess individuals and families, provide evidence-based treatment, and collaborate with the physician and care team to ensure timely access to services.

Qualifications

• MSW, RN, BScN, CPMHN(C) or equivalent.
• College/professional association membership.
• Experience with children/youth, families and perinatal mental health.
• Minimum 3 years' experience assessing and managing mental health problems in primary care.
• Formal training and experience in IPT and CBT.
• Ability to complete risk assessment and manage crisis.
• Knowledge of chronic disease prevention and CDPM is an asset.
• Familiarity with privacy legislation and HFHT policies.

Responsibilities

• Conduct comprehensive assessments for individuals, couples and families.
• Provide evidence-based treatment and management of mental health problems.
• Implement current best practice guidelines and monitor progress.
• Coordinate referrals and access to community programs and services.
• Liaise with the health care team and navigate care pathways.
• Document care clearly and timely in EMR systems.
• Participate in continuing education and program evaluation.
