The successful candidate’s primary focus will support early management of Chronic Kidney Disease (CKD) which includes CKD identification, prevention, and the concepts of patient self-care management. Responsibilities also include patient screening, preparation and promotion of independent modalities. These responsibilities will be in collaboration with the Nephrologists, Allied team and the CKD clinic nurses; within a mentorship environment. The candidate may be required to work at the Guelph Renal Clinic. Contributes to ensuring there is a safe environment for patients, staff and visitors.
