Job summary

VHA Home HealthCare in Ontario is seeking a Registered Practical Nurse to provide and coordinate home health nursing services to diverse clients in their homes and community settings. You will assess, monitor, and evaluate care plans, document thoroughly, and collaborate with a supportive team to ensure high-quality, client-centered care and ongoing professional growth.

Grounded in evidence-based practice and driven by compassion, this role offers meaningful daily impact with flexible scheduling

Qualifications

• Registered and in good standing with the College of Nurses of Ontario (CNO).
• Valid First Aid or CPR Certification.
• Acute Care and Medical-Surgical nursing experience is an asset.
• A valid Ontario driver’s license (G License) and a reliable vehicle for traveling are required.

• Ensure quality of patient care by identifying and addressing factors affecting practice.
