VHA Home HealthCare is seeking registered nurses to deliver family-centered home health nursing across Ontario. You will assess, plan, and coordinate care for clients in homes, schools, and community settings while upholding nursing standards.

Qualifications

• Registered with the College of Nurses of Ontario (CNO) in good standing.
• RN diploma/degree from a recognized university or college.
• Hold a valid First Aid or CPR Certification.
• Acute Care and Medical-Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Demonstrate effective communication (written and verbal) and documentation skills.
• Demonstrate reliable attendance, punctuality, and availability for the role.
• A valid Ontario driver’s license (G) and a reliable vehicle for traveling are required.

Responsibilities

• Assess, monitor, and coordinate care plans.
• Evaluate care plan interventions through ongoing reassessment and evaluation results.
• Ensure quality of care and professional accountability in interactions with clients and families.
• Document thoroughly and inform the nursing supervisor of any changes in the patient plan.
• Support client safety and continuous quality improvement initiatives.
• Communicate changes to the case coordinator and maintain required records.

Actual earnings may vary based on availability and service volumesGrounded in our commitment to Client and Family Centred Care the key foci on the RN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.The Child and Family Program provide individualized, one-on-one care to clients ranging from newborns to young adults up to 25 years of age. As a visiting nurse, you will deliver home-based nursing care to children with complex medical needs, supporting both their health and overall well-being in a family-centered environment.
