Job summary

VHA Home HealthCare in Durham Region, ON is seeking a part-time Registered Practical Nurse to provide and coordinate home health nursing services in diverse client settings. You will assess and monitor clients, develop care plans, and collaborate with families and the care team to promote independence at home.

The role offers per-visit compensation with an additional top-up premium for in-person nursing visits, and an estimated annual earning potential of CAD 70,000–120,000 depending on service

Qualifications

• Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO).
• Membership in a professional association preferred (RNAO/WeRPN).
• CPR/First Aid certification required.
• Acute care or Medical-Surgical nursing experience is an asset.

Responsibilities

• Assess, monitor and coordinate care plans for clients.
• Reassess and evaluate care plan effectiveness regularly.
• Maintain high quality patient care and professional accountability.
• Document and communicate client status to the nursing supervisor.
• Support safety and continuous quality improvement in care.
