Qualifications

• Registered with the College of Nurses of Ontario in good standing.
• Experience in Home and Community Care is an asset.
• Current BCLS certification (Level C).
• Demonstrated clinical assessment and patient/family teaching skills.
• Demonstrated effective communication skills.
• Charting on Procura is an asset.

Responsibilities

• Provide nursing care in home and community settings.
• Assess patients and educate families on care plans.
• Coordinate with the care team and document care in charting.
• Support peritoneal dialysis, wound care, IV management, and diabetic care in community settings.

Core Competencies:

• Registered with the College of Nurses of Ontario (CNO) and in good standing
• Experienced in Home and Community Care is an asset
• Current BCLS certification (Level C)
• Demonstrated clinical assessment and patient/family teaching skills
• Demonstrated effective communication skills
• Charting on Procura is an asset

Shifts: 3 days during the week and alternate weekends

Requirements:

• Clear & updated Vulnerable Sector Check (within last six months)
• Valid Driver's License & reliable car in community homecare is required
• Up to date TB test within last 2 years
• Must have your own:
• Stethoscope
• Blood pressure cuff and Sphygmomanometer

Note:

This is a specialty program for Peritoneal Dialysis, Wound Care, IV Management, Diabetic Care & Post Operative Care in the community.
