Job summary

VHA Home HealthCare is seeking Registered Practical Nurses (RPNs) and Registered Nurses (RNs) to support the Palliative care program in Durham region. This visiting nurse role requires a valid Ontario driver’s license and access to a reliable vehicle for travel to clients’ homes.

The program delivers end‑of‑life care and collaborates with external palliative care teams, care coordinators, physicians, and PSWs. Excellent communication and documentation skills are essential.

Qualifications

• Registered and in good standing with the College of Nurses of Ontario (CNO).
• RPN or RN diploma/degree from a recognized university or college.
• Valid Ontario driver’s license (G license) and access to a reliable vehicle.
• First Aid or CPR certification.
• Wound care or venipuncture knowledge is an asset.
• Acute Care/Medical‑Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Strong communication and documentation skills.

Responsibilities

• Assess, monitor, and coordinate care plans for diverse clients.
• Reassess and evaluate interventions to ensure quality care.
• Maintain documentation according to professional standards and policies.
• Demonstrate professionalism, leadership, and accountability in client interactions.
• Support safe client care through ongoing education and quality improvement.
• Inform the nursing supervisor of changes in client status and document with case coordinators.
• Participate in standby/on-call rotation and team meetings as required.

Pay Range

Rate: $26.60-$41.10 /visit (placement on salary range as per Collective Agreement), plus palliative visit stipend

Estimated annual earning potential: $80,000–$130,000. Actual earnings may vary based on availability and service volumes

Grounded in our commitment to Client and Family Centred Care the key foci on the RN and RPN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.

We are currently seeking Registered Practical Nurse (RPNs) and Registered Nurses (RNs) to support our Palliative care program in Durham region. This is a Visiting Nurse position, candidates must possess a valid Ontario driver's license (G License) and have access to a reliable vehicle for traveling.

The Palliative care program delivers home nursing and end-of-life care for individuals, families, and groups. Palliative nurses collaborate inter-professionally with external palliative care community teams, care coordinators, physicians, nurse practitioners, and personal support workers (PSWs).

Key Areas of Accountability

• Assess, monitor, and make care planning and care coordination.
• Evaluate care plan interventions through on-going reassessment and evaluation results.
• Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.
• Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
• Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
• Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
• Maintain documentation standards according to the College of Nurses of Ontario.
• Participate in a standby/on‑call rotation to be available for answering calls and respond, if needed.
• Participate in team meetings, as required during the stated availability.

Required Qualifications

• Registered and in good standing with the College of Nurses of Ontario (CNO).
• Membership of your Professional Association preferred (RNAO/WeRPN).
• RPN diploma or RN diploma/degree from a recognized university or college.
• Hold a valid First Aid or CPR Certification.
• A moderate level of wound care or venipuncture knowledge and experience is an asset.
• Acute Care and Medical‑Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Demonstrate effective communication (written and verbal) and documentation skills.
• Demonstrate reliable attendance, punctuality, and availability for the role.
• Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
• A valid Ontario driver’s license (G License) and a reliable vehicle for traveling are required.
