Job summary

VHA Home HealthCare in Ontario is seeking Registered Nurse or Registered Practical Nurse to provide and coordinate home health nursing services across diverse clients in North York and Toronto. The role requires morning and day availability with bi-weekly weekend rotation, and a valid G license with a reliable vehicle for travel.

As a Visiting Nurse, you will assess patient needs, implement care plans, and collaborate with families and a regional team to ensure high-quality, client-centered care

Qualifications

• Registration in good standing with the College of Nurses of Ontario (CNO).
• RPN or RN diploma/degree from a recognized college or university.
• First Aid or CPR Certification.
• Knowledge of wound care or venipuncture is an asset.
• Acute Care and Medical-Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Strong written and verbal communication and documentation skills.
• Reliable attendance, punctuality and availability for the role.
• Ontario driver's license (G) and a reliable vehicle for traveling.

Responsibilities

• Evaluate care plan interventions through ongoing reassessment and evaluation results.
• Ensure quality of patient care by addressing factors affecting practice autonomy and delivery of care.
• Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
• Support safe client care through knowledge and skills aligned with continuous quality improvement.
• Inform nursing supervisor of client activities, concerns, or changes, and document appropriately to the case coordinator, maintaining College of Nurses of Ontario documentation standards.

Pay Range

• Rate: $26.60 - $41.10 /visit (placement on salary range as per Collective Agreement)
• Estimated annual earning potential: $80,000-$130,000. Actual earnings may vary based on availability and service volumes

Grounded in our commitment to Client and Family Centred Care the key foci on the RN and RPN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.

ISDS brings VHA's community programs together within one coordinated system to strengthen integrated care, promote collaboration across disciplines, and provide clients with a seamless care experience. The successful candidate will work in the North York and Toronto areas. We are seeking a Registered Nurse or Registered Practical Nurse with morning and day availability, and bi-weekly weekend rotation (from 8:00 a.m.-4:00 p.m. or 9:00 a.m.-5:00 p.m.). As a Visiting Nurse, a reliable vehicle and a valid G2 or G driver's license are required for this role.

Key Areas of Accountability

• Assess, monitor, and make care planning and care coordination.
• Evaluate care plan interventions through on-going reassessment and evaluation results.
• Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.
• Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
• Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
• Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client's condition or care plan, and submit appropriate documentation to the case coordinator.Maintain documentation standards according to the College of Nurses of Ontario.

Required Qualifications:

• Registered and in good standing with the College of Nurses of Ontario (CNO).
• Membership of your Professional Association preferred (RNAO/WeRPN).
• RPN diploma or RN diploma/degree from a recognized college or university.
• Hold a valid First Aid or CPR Certification.
• A moderate level of wound care or venipuncture knowledge and experience is an asset.
• Acute Care and Medical-Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Demonstrate effective communication (written and verbal) and documentation skills.
• Demonstrate reliable attendance, punctuality, and availability for the role.
• Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
• A valid Ontario driver's license (G License) and a reliable vehicle for traveling are required.
