Job summary

VHA Home HealthCare (VHA) is seeking a casual Registered Nurse to support the Palliative Care Program in Durham North. The RN will provide and coordinate home health nursing services, working with diverse clients and families.

A valid Ontario driver’s license and reliable vehicle are required for travel between communities. The role delivers end‑of‑life care and requires strong communication, documentation, and collaboration with inter‑professional teams.

Qualifications

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Membership in RNAO preferred.
  • Valid First Aid or CPR Certification.
  • Acute Care/Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.

Responsibilities

  • Assess, monitor, and make care planning and care coordination.
  • Evaluate care plan interventions through ongoing reassessment and evaluation.
  • Ensure the quality of patient care by identifying and acting on factors which affect practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
  • Support safe client care through development of knowledge and skills in quality improvement.
  • Inform the nursing supervisor of client activities or changes and submit documentation.
  • Maintain documentation standards per College of Nurses of Ontario.
  • Participate in standby/on‑call rotation and team meetings.

Rate: $33.53 - $41.10 /visit (placement on salary range as per Collective Agreement), plus palliative visit stipend


Estimated annual earning potential: $80,000–$130,000. Actual earnings may vary based on availability and service volumes


Grounded in our commitment to Client and Family Centred Care the key foci on the RN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.


We are currently seeking a casual Registered Nurse (RN) to support our Palliative care program in Durham North. This is a Visiting Nurse position, candidates must possess a valid Ontario driver's license (G License) and have access to a reliable vehicle for traveling.


The Palliative care program delivers home nursing and end-of-life care for individuals, families, and groups. Palliative nurses collaborate inter-professionally with external palliative care community teams, care coordinators, physicians, nurse practitioners, and personal support workers (PSWs).


Key Areas of Accountability

VHA's Registered Nurses are required to do the following tasks, but not limited to:


Assess, monitor, and make care planning and care coordination.


Evaluate care plan interventions through on-going reassessment and evaluation results.


Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.


Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.


Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.


Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.


Maintain documentation standards according to the College of Nurses of Ontario.


Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.


Participate in team meetings, as required during the stated availability.


Required Qualifications:

Registered and in good standing with the College of Nurses of Ontario (CNO).


Membership of your Professional Association preferred (RNAO).


RN diploma or degree from a recognized college or university.


Valid First Aid or CPR Certification.


Acute Care and Medical-Surgical nursing experience is an asset.


Knowledge of community nursing is an asset.


Demonstrate effective communication (written and verbal) and documentation skills.


Demonstrate reliable attendance, punctuality, and availability for the role.


Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.


A valid Ontario driver’s license (G License) and a reliable vehicle for traveling are required.
