VHA Home HealthCare in Ontario is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) for the Integrated Service Delivery System (ISDS) Program, focused on project management. Rate: $26.60 - $41.10 /visit (placement on salary range as per Collective Agreement)

ISDS brings VHA's community programs together within one coordinated system to strengthen integrated care, promote collaboration across disciplines, and provide clients with a seamless care experience. The successful candidate will work in the North York and Toronto areas. We are seeking a Registered Nurse or Registered Practical Nurse with morning and day availability, and bi-weekly weekend rotation (from 8:00 a.m.-4:00 p.m. or 9:00 a.m.-5:00 p.m.). As a Visiting Nurse, a reliable vehicle and a valid G2 or G driver's license are required for this role.
