Job summary

• Provide nursing care in accordance with organizational policies, guidelines and evidence-based practice.
• Collaborate with clients and health-care teams to coordinate services across settings.
• Deliver client-centered care focusing on health promotion, prevention and palliation as needed.
• Utilize community programs and services knowledge to support care planning.
