Job summary

Vancouver Island Health Authority is seeking a Registered Nurse to join Primary Care across UPCC and Family Physician/Nurse Practitioner clinics. The role emphasizes client-centered care, health coaching, and coordination with interdisciplinary teams to provide episodic and longitudinal primary care for unattached and attached clients of all ages.

The RN will perform initial assessments, promote health, support self-management, and facilitate seamless transitions within the care continuum,

Qualifications

• Registration with BC College of Nurses and Midwives as an RN.
• Two years of recent nursing experience in primary care, outpatient or similar settings.
• Valid BC driver’s license and Basic Life Support Level C.

Responsibilities

• Provide direct patient care in UPCC, Patient Medical Homes or primary care clinics.
• Collaborate with an inter-professional team to coordinate care.
• Promote health education and self-management with clients.
• Facilitate client transitions across the healthcare continuum.
• Adhere to practice standards, ethics, and cultural safety guidelines.
