Reporting to the Manager, Quality & Risk, this position serves as a key organizational resource for risk management and patient safety issues.

What will you do?

• Embody Ontario Health atHome’s mission, vision, and values, applying the quadruple aim in daily work
• Use quality improvement tools to enhance care and foster a culture of continuous improvement

• Support equity, inclusion, diversity, and anti-racism initiatives
• Act as a resource for risk management and patient safety issues
• Provide advice on high-risk situations and complex cases
• Respond to urgent risk management and patient safety issues, consulting with stakeholders as needed
• Assist clinical and non-clinical teams in implementing risk mitigation strategies
• Support initiatives with external organizations (e.g., HIROC, Ontario Public Health)
• Promote a safety and risk awareness culture
• Contribute to risk management tools and policies, including incident reviews and root cause analyses
• Participate in ongoing education related to risk and patient safety
• Provide training on risk management and infection prevention
• Engage in projects and working groups as assigned
• Support legal claims processes

What must you have?

• Undergraduate degree
• Risk Management certification
• Knowledge of relevant legislation (e.g., Home Care Act, PHIPA)
• Experience in quality and risk projects
• Understanding of Ontario’s healthcare system and community service models
• Project management skills
• Knowledge of emergency preparedness and incident management
• High computer literacy and presentation skills
• Understanding of patient-centered care
• Facilitation and interpersonal skills, including conflict resolution and emotional intelligence
• Ability to handle high-risk situations calmly
• Strong writing skills for reports and analyses
• Ability to analyze complex information and develop risk mitigation strategies
• Proactive promotion of risk mitigation strategies
• Experience advising management on risk strategies
