Job summary

Connective is a dynamic non-profit organization providing supportive housing and social services. This role will deliver casework to residents to enhance quality of life, ensure safe operation of the facility, and support rehabilitation plans.

You will work with a multidisciplinary team, facilitate daily living skills, coordinate with community resources, and document progress while upholding policies and safety standards.

Qualifications

• Diploma or equivalent in a human/social service field.
• At least 1 year experience with homelessness, substance use, mental health, or housing barriers.
• Experience with First Nation communities is a plus.

Responsibilities

• Participate in assessment, goal setting, and program planning for residents.
• Evaluate client needs and develop short-term plans with resident and family input.
• Work with social workers, outreach staff, and community partners on support plans.
• Assist residents with daily living skills, activities, and safe decision-making.
• Document progress and provide feedback to residents and families.
