Job summary

Connective, a Kelowna-based nonprofit, seeks an External Tenant Support Worker to provide casework and support for residents in Connective programs. The role focuses on enhancing quality of life, ensuring safe operation of the facility, and maintaining counselling/rehabilitative goals.

Working in a unionized environment, you will coordinate with care teams, liaise with community partners, and document progress while upholding health and safety standards.

Qualifications

• Diploma in a related human/social service field.
• At least 1 year of related experience.
• Excellent written and verbal communication in English.

Responsibilities

• Participates in assessment, goal setting, and program planning for individuals living semi independently.
• Evaluates client needs and develops short-term plans with resident participation.
• Collaborates with Social Workers, Outreach Workers, and community partners to inform support plans.
• Assists residents with daily life skills and community involvement to promote independence.
• Documents progress and provides feedback to residents and families; maintains required records.
• Transports clients to appointments or activities as needed.
