Job summary

Sienna Senior Living is seeking a Resident & Family Experience Coordinator III to lead move-in/out processes and support families, residents and SDMs with transitions and access to supports. The role uses social work principles to enhance wellbeing in long-term care and coordinates with the inter-professional team.

You will provide counselling, develop care plans, support advance care conversations, recruit and supervise social work students, and report feedback to Leadership & Quality

Qualifications

  • Registered social worker in good standing per provincial legislation.
  • Minimum three years’ experience in healthcare, social services, or related sector.
  • Knowledge of Long Term Care Act & Regulations preferred.
  • Customer service experience an asset.
  • Hospitality or sales related experience an asset.
  • Strong oral and written communication skills.
  • Ability to organize, plan, and delegate responsibilities.

Responsibilities

The Resident & Family Experience Coordinator III is a critical role within our communities as it impacts the lives of residents and ignites the warmth of human connection.

As a Resident & Family Experience Coordinator III, your typical day will impact the following ways:

  • Leads the resident move in, transfer, and move out process in accordance with the organization’s move in experience standards to include: facilitation of the application

process including assistance to access financial supports as required, tours, introducing the resident/family/substitute decision maker (SDM) to the community and its operations, assists and supports resident/family/SDM with adjusting to living in long term care

  • Provides social work counselling services as required, working with the inter-professional team to assess and develop a plan of care that meets residents’ psychosocial and social needs, promoting the resident’s overall wellbeing and safety.
  • Plan, develop, implement, and evaluate individual social work services and/or support groups to meet the identified needs of residents and families. Engage external resources as required to ensure resident social needs are met
  • Supports residents and families during advanced care planning and treatment plan conversations within Social Services Worker scope, including funeral arrangement discussions
  • Recruit and supervise social work students’ placements
  • Responsible for response, correspondence & tracking of feedback received in the resident home area/neighborhood to ensure they are addressed in a timely manner and receive satisfactory outcomes.
  • Responsible for providing reporting and analysis, feedback received to Leadership & Quality Committee
Must Haves
  • Social Worker; must be Registered per provincial legislation
  • Minimum of three years’ experience in healthcare, social services, or related sector
  • Knowledge of the Long Term Care Act & Regulations preferred
  • Customer service related experience an asset
  • Hospitality and/or sales related experience an asset
  • Strong oral and written communication skills required

Leveling rate of pay $42.27 - $52.81 per hour until such time that wage leveling ends, then the pay rate will reflect the collective agreement pay rates starting rate
