Job summary

AgeCare is seeking a Licensed Practical Nurse (LPN) to provide planned, patient-oriented nursing care in a long-term care setting. The role reports to the Director of Care and requires current LPN registration in the applicable province, strong clinical knowledge, and the ability to document care in accordance with regulatory standards.

Responsibilities include completing resident assessments and care plans, implementing and evaluating care, and coordinating care conferences.

Qualifications

• Current registration as a Licensed Practical Nurse in the province.
• Experience in a senior environment; unregulated care providers preferred.
• Education/experience in gerontology or rehabilitation is an asset.
• Strong written and oral communication, critical thinking, care coordination, time management.
• Sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification.
• Computer proficiency with Microsoft Office.
• Vulnerable Sector Check within six months or willingness to obtain one.

Responsibilities

• Provide planned, patient-oriented nursing care in accordance with provincial standards.
• Complete and ensure resident care assessments, care/service plans and documentation are accurate and current.
• Implement and evaluate care/service plans and adjust as needed.
• Assist with scheduling resident care conferences and participate when applicable.
• Collaborate with the Director of Care on move-in/move-out and transfers.
• Provide direct resident care and medication/treatment administration with proper documentation.
