Qualifications

• Minimum of 2 years of front-line clinical hospital experience as a pharmacist.
• Ability to make decisions while adhering to site-specific policies, procedures, and practices.

Responsibilities

• Provide drug order entry and/or review.
• Review medications to ensure clinical appropriateness.
• Identify, assess and resolve drug-related problems.
• Complete medication reconciliation.
• Provide medication information and/or advice in a clear and concise manner.

Skills

Bilingual in French and English

Excellent written and verbal communication skills

Ability to work efficiently and independently

Computer Skills (Word, Excel, PowerPoint, Outlook, Adobe)

Education

Licensed Pharmacist in Good Standing

Hospital residency or M.Sc. in advanced pharmacotherapy
