Job summary

CarePartners is seeking a Personal Support Manager to coordinate and supervise the delivery of Community Health Services in the Quinte area. You will work from the Quinte West office with occasional time in Brockville, ON, and participate in an on-call rotation that includes evenings and weekends.

The role is full-time with a competitive salary and benefits, plus a $1,500 signing bonus after six months. Flexible work-from-home arrangements support work-life balance while you lead health care

Qualifications

  • RN or RPN degree or diploma is required.
  • CNO registration is required.
  • 3–5 years of related experience.
  • Vehicle is required and some travel is expected.
  • English fluency is essential; French is an asset.
  • CPR certification is required.
  • Background and Vulnerable Sector Check is required.

Responsibilities

  • Provides coordination, supervision, and coaching to Community Support Workers in the field.
  • Develops and maintains an effective Care Plan with respect to Community Health Services, independently or with Ontario Health atHome.

We are currently looking for someone to join our dynamic Leadership Team in the role of Personal Support Manager for the Quinte area. The primary role of the Personal Support Manager is to provide effective coordination and supervision of the delivery of Community Health Services.

This is a Full-Time position. Hours of work will be Monday-Friday as well as participation in the Branch Manager On-Call, which includes evenings and weekends on a rotational basis.

Will be working out of the Quinte West office, with time in Brockville, ON branch from time to time.

  • This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $64,935.00 to $82,875.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What You Bring
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • 3 - 5 years of job-related experience or an equivalent combination of education and experience
  • Registration with the College of Nurses of Ontario (CNO)
  • Experience in a community healthcare environment considered an asset
  • Experience in a formal leadership role considered an asset
  • Proven computer skills, including email, word processing, work scheduling systems
  • Thorough knowledge of case management skills and nursing processes and practices
  • Vehicle is required and some regular travel is required
  • Leadership and people management skills are an asset
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Current CPR Certification
  • Clear Background and Vulnerable Sector Check
