Job summary

CMHA Peel Dufferin is seeking a Relief Outreach Worker to join the Outreach team, providing housing-first, client-centered support to individuals experiencing homelessness or at risk in the Peel Region. This on-site role emphasizes relationship-building, trust, and empowerment to help clients access permanent housing and essential services.

The position involves 12-hour shifts on weekdays, evenings, and weekends, with a base hourly salary of $32.65–$37.66.

Qualifications

• Post-secondary degree or diploma in relevant discipline (e.g. BSW, MSW, OT, RN).
• Minimum of 3 years of experience supporting those experiencing homelessness in a community setting.
• Experience working in an outreach setting with strong understanding of Outreach best practices is an asset.
• Previous case management experience is preferred.
• Personal experience with mental health, addiction and/or homelessness is an asset.

Responsibilities

• Build client capacity and create self-reliance.
• Develop relationships with prospective clients based on trust and honesty.
• Assist with securing/matching affordable housing for those at risk or homeless in Peel Region.
• Support housing transitions using a Housing-Based Street Outreach approach and document plans.
• Complete intake and discharge assessments as required.
• Conduct case conferencing with support networks.
• Maintain relationships with referral sources and service providers.
• Conduct weekly community/home visits to offer support and resources.
• Advocate with participants on landlord relations and tenancy issues.
• Collaborate with Peel Region Housing Stability workers on life skills to maintain housing.
• Maintain client records and CMHA/Peel documentation.
