Fraser Health Authority in White Rock, BC is seeking a Relief Full Time Community Health Nurse - Registered Nurse to join our team in the community setting.

This temporary relief role is expected to continue until April 2028 or until the incumbent returns, offering exposure to chronic disease management, home visits and collaboration with an interprofessional team. Formación

• Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Minimum 1 year of clinical experience with geriatric or adult populations in acute or community settings.
• Valid BC driver’s license and access to a personal vehicle for business purposes.
• Experience in home/community nursing is an asset.

Responsabilidades

• Establish a therapeutic relationship with clients to support autonomy in decision-making and care planning.
• Conduct client assessments and interprofessional care planning with collaboration across teams.
• Develop and support comprehensive care plans and transitions to other services.
• Provide direct client care in home, clinic or community settings.
• Coordinate tasks with Community Health Workers and provide in-home training as needed.
• Facilitate care conferences and monitor care plan effectiveness.
• Promote health education, self-management and disease management for clients and families.
• Maintain accurate clinical records and contribute to quality improvement initiatives.
