Qualifications

• Bachelor's degree in a health-related discipline (e.g., Occupational Therapy, Physiotherapy, Social Work).
• Registration in good standing with relevant professional college or regulatory body in Ontario.
• Case Management Certificate preferred.
• One year of recent experience in case management and discharge planning.
• Three years of recent clinical experience with geriatric or rehab populations preferred.

Responsibilities

• Provide first-line facilitation between patients, families, physicians, nursing and allied staff to ensure seamless transitions of care.
• Assess discharge and transition needs of patients and families.
• Educate patients and families on coping with lifestyle changes.
• Coordinate and develop an integrated discharge plan in a safe and timely manner.
• Demonstrate knowledge of relevant legislation (e.g., Health Care Consent Act, Mental Health Act).
