Qualifications

• Case Management Certificate preferred.
• One year of recent experience in case management and discharge planning.
• Three years of recent clinical experience with geriatric or rehab populations preferred.

Responsibilities

• Provide first-line facilitation between patients, families, physicians, nursing and allied staff to ensure seamless transitions of care.
• Assess discharge and transition needs of patients and families.
• Educate patients and families on coping with lifestyle changes.
• Coordinate and develop an integrated discharge plan in a safe and timely manner.

The Case Manager assists in setting realistic goals and developing coping strategies, which helps to create a seamless continuum of care for patients. The incumbent coordinates and develops an integrated discharge plan in a safe and timely manner as well as provides education, insight and consultation in accessing resources, linking to appropriate community and informal supports.

The Case Manager’s scope of practice is broad including individual, and family counseling to assist patients/clients and families who are experiencing emotional difficulties, behavioral changes, family conflicts, grieving losses, adjustment to changes in life style, difficulty in accessing community resources, financial problems and discharge planning.

The successful candidate will:

• Provide first line facilitation between patients, their families/caregivers, physicians, nursing and allied staff, creating seamless transitions of care through the health care continuum.
• Possess skills to assess the discharge and transition needs of patients and their families.
• Educate patients and families regarding strategies related to coping to lifestyle changes.
• Demonstrate knowledge and understanding of ALC, patient care flow process or community resources.
• Effectively work with the interdisciplinary team to coordinate and develop an integrated discharge plan in a safe and timely manner.
• Possess knowledge of relevant legislation (i.e. Health Care Consent Act, Substitute Decision Act, Mental Health Act, Public Guardian and Trustee, Child Protection Act).
• Demonstrate commitment to patient/family centered care.
• Demonstrated professionalism and community relations skills while dealing with the public (patients and families), as well as various medical and non-medical professionals.
• Possess effective organizational, communication, negotiation and leadership skills in decision-making.
• Integrate education, critical thinking, and relevant practical experience in problem solving.
• Possess human relations, empathy and interpersonal skills sufficient for successful conflict resolution and to build and maintain positive relationships with patients, families, and hospital staff; and develop effective partnerships with community agencies/facilities.
• Be proficient English language literacy skills sufficient to read and understand all relevant information, investigate available resources, and communicate through written and verbal exchanges with internal and external contacts.
• Be self-directed and able to coordinate and prioritize multiple competing workload demands with minimal supervision.
• Apply HDSHRC values and professional practice standards to patient/family care and service.
• Performs other duties as required.for the provision of clinical and relevant drug distribution pharmacy services with the purpose of optimizing outcomes to improve the patients’ quality of life,

The successful candidate will possess:

• A Bachelor's degree in a health-related discipline (for example: Occupational Therapy, Physiotherapy, Social Work).
• Registration in good standing with relevant professional college or regulatory body in Ontario.
• Case Management Certificate preferred.
• One (1) year of recent experience in case management and discharge planning required, preferably in a hospital environment.
• Three (3) years of recent clinical experience with geriatric or rehab populations preferred.
• Proficiency in both official languages (English and French) will be considered an asset.
