Job summary

Hotel Dieu Shaver Health and Rehabilitation Centre in St. Catharines, Ontario, is seeking a Pharmacist for a casual position.

You will join a dedicated pharmacy team responsible for safe drug distribution, medication reviews, and patient education within an interprofessional setting. The successful candidate will hold a Bachelor of Science in Pharmacy, be licensed in Ontario (Part A), and ideally have hospital experience; bilingual English/French is a plus.

Qualifications

• Registered pharmacist in Ontario with Part A license.
• Bachelor of Science in Pharmacy required; PharmD an asset.
• Hospital experience preferred; ability to work in a multidisciplinary team.

Pharmacy Dept. plays a critical role in ensuring safe drug distribution and optimum clinical care to our patients in compliance with the legal, regulatory and accreditation standards. As a part of pharmacy team and a key member of multidisciplinary team, Pharmacist is responsible for the provision of pharmaceutical care and range of clinical services to our patients, and provide medication related consultations and education to the patients, staff and care providers.

This position reports to the Director of Pharmacy and Quality Improvement.

Position Responsibilities

Includes, but not limited to:

• Responsible for the provision of clinical and relevant drug distribution pharmacy services with the purpose of optimizing outcomes to improve the patients’ quality of life,
• Direct, patient-centred care in partnership with patients/family in a multi-disciplinary team environment to address drug therapy issues, including patient interviews for performing best possible medication histories and medication reviews for reconciliation activities,
• Perform medication and polypharmacy reviews, participate in quality improvement activities, engage in incidence and adverse event reporting, provide drug information and effectively collaborate with other healthcare providers to promote evidence-based practices,
• Order entry and verification using critical thinking and expertise to provide therapeutic check for safe drug distribution in Oracle Cerner Provider Order Entry (CPOE) environment,
• Performs the duties of clinical preceptor or tutor, as needed,
• Other duties as assigned.

Qualifications

• Bachelor of Science in Pharmacy is required, Pharm D an asset.
• Current registration with the Ontario College of Pharmacists required – Part A.
• Previous hospital experience, preferred. Relevant advance certification or Master’s degree qualification will be an asset.
• Demonstrated ability to work independently and as a member of a team.
• Demonstrated ability to prioritize a consistent and varied workload.
• Ability to direct and teach staff.
• Demonstrated written and verbal communication skills required, to facilitate effective communication with patients, peers, other health care providers, and volunteers.
• Demonstrated ability to identify, prevent and resolve drug related problems.
• Demonstrated excellent attendance and discipline free record required.
• Good understanding of change management; must be able to identify change needs and proactively act on those needs.
• Effective negotiation and conflict management/ resolution skills required.
• Working knowledge of use of computers and information technology (e.g. Windows Operating System, Microsoft Office), hospital/ Pharmacy information system and closed loop medication management systems is required.
• Prior experience with Oracle Cerner, automated packager PACMED and PACVISION, Medidose, BD Pyxis automated dispensing cabinets is preferred.
• Proficiency in both official languages (English and French) will be considered an asset.
