Responsibilities

• Maintain knowledge of relevant legislation including Mental Health Act and related acts.

• Coordinate service delivery and collaborate with patients, families and healthcare teams.
• Conduct comprehensive psychosocial assessments from a systems perspective.
• Identify and intervene with high risk social situations as appropriate.
• Provide case management and facilitate complex discharges.
• Lead discharge planning and mentor new staff as required.
• Meet Ontario Social Work recertification requirements and ensure data reporting.
