Job summary

ParaMed in Ontario is seeking a Registered Practical Nurse (RPN) for part-time community visits. You will deliver ethical, safe nursing care, assess patients, and follow care plans with support from RNs for complex cases.

Travel to patient residences within your designated area is required, and you will build trusting relationships in the home setting. This role offers autonomy, flexible scheduling including every other weekend off, comprehensive health and dental benefits, education allowances,

Qualifications

• Current license as RPN with CNO in good standing.
• Valid driver’s license and vehicle access.
• Experience in community/home care is an asset.
• Reliable and flexible with willingness to travel.
• Comfort working with vulnerable populations and on-call coverage.

Responsibilities

• Participate in scheduled community visits to assess patient needs.
• Provide ethical, safe nursing care within scope of practice.
• Use assessment tools to tailor patient care plans.
• Execute and adjust care plans as required.
• Build rapport/trust with patients and maintain records.

In this community nursing role, you as a Registered Practical Nurse (RPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. What you’ll do:

• Participate in scheduled visits in the community to look after patient needs within your scope of practice
• Provide ethical and safe nursing care
• Use appropriate assessment tools/techniques to assess patients’ individual needs
• Execute and continuously monitor care plans and adjust as required
• Engage patients using a collaborative approach by building rapport and trust
• Maintain and update professional patient records related to patient encounters and plans

What you bring:

• Registration with the CNO as a Registered Practical Nurse in good standing
• Valid driver’s license and access to a vehicle is required
• Experience in delivering community care is an asset
• Reliability and flexibility
• Willingness to travel to and from patient residences within a designated area
• Experience working with the vulnerable population

• Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
