Job summary

Southbridge Care Homes in Etobicoke is seeking a Registered Practical Nurse - BSO for a temporary full-time role (8/10/2026-10/30/2027) with day shifts from 9am to 5pm. The RPN BSO provides specialized behavioral support to residents, applying expertise in dementia care, mental health, and responsive behaviors.

Qualifications

  • Current certificate of competence from the College of Nurses of Ontario.
  • Minimum two years of experience in a similar role, preferably long-term care.
  • Degree in human psychology or behavioral sciences is an asset.
  • Certification in behavioural training (P.I.E.C.E.S, UFirst, GPA).
  • A serving heart and a passion for caring for seniors.
  • Registration/eligible for registration with the RPN Ontario Association.
  • Ability to work independently and in a team, prioritize tasks.
  • Excellent communication and comprehension skills.
  • Creative problem solving to view the big picture.

Responsibilities

  • Contribute to person-centered care and promote residents' quality of life.
  • Conduct assessments and identify behavioral risks with caregiver strategies.
  • Ensure care plans address responsive behaviors and safety.
  • Communicate with external stakeholders and management about risks.
  • Participate in quality improvement activities and monthly reports.
  • Provide coaching and mentorship to staff for coordinated care.
  • Attend quality program meetings and care reviews.
  • Collaborate with pharmacists and physicians regarding medications.
  • Participate in staff development and ongoing training.
  • Support administrative duties and interdepartmental cooperation.

Reports to: Director of Care

The RPN BSO Provides Specialized Behavioral Support To Residents, Applying Expertise In Dementia Care, Mental Health, And Responsive Behaviors. They Assess And Implement Individualized Strategies To Manage Behavioral And Psychological Symptoms, Working Closely With Residents, Families, And The Care Team. The RPN BSO Coordinates With Multiple Stakeholders To Develop And Maintain a Person-centered Philosophy Of Care. They Also Support The LTC Home In Medication Administration, Treatments, And Assessments, And Actively Participate In Multidisciplinary Meetings To Promote Resident Well-being And Uphold High Standards Of Nursing Practice

Key Responsibilities
  • Contribute in delivering person-centered care by promoting the abilities and self-esteem of the residents, develop strategies to promote optimal quality of life for the residents with the responsive behaviors.
  • Conduct assessments using various assessment tools which describes behaviors of residents including observational behaviors, identifying behavioral risks, and identify caregiver strategies.
  • Ensure the plan of care is regulated with responsive behaviors of residents in regard to the staff providing care and mitigating risks by conducting weekly rounds.
  • Communicate with the external stakeholder and management team to identify the risks associated with responsive behavior.
  • Demonstrate participation in quality improvement activities and provide monthly reports.
  • Provide coaching and mentorship to the staff to ensure collaboration of care and services for the responsive behavior.
  • Conduct resident care reviews and attend quality program meetings including P.A.C.
  • Collaborate with the pharmacists and physicians for the consultation regarding the affect of medication treatment on residents.
  • Participate in the staff development programs and seminars and require attending ongoing training to support skills in managing residents with responsive behaviors.
  • Assist in tasks by considering policies of the home and the behavioral support Ontario project.
  • Maintain amazing relationship with all the stakeholders including staff, residents and the general public.
  • Provide an excellent customer service to all the stakeholders and assist in the administrative duties of the home.
  • Other duties as required.
Knowledge And Skills Required
  • Appropriate current certificate of competence from the College of Nurses of Ontario.
  • Minimum two years of experience required in the similar role, preferably in the long-term care setting, with comprehensive knowledge of duties.
  • Degree in human psychology, or behavioral sciences would be considered as an asset
  • Certification in behavioural training (P.I.E.C.E.S, UFirst, GPA etc.)
  • A serving heart and a passion for caring for seniors.
  • Registration/eligible for registration with the Registered Practical Nurses Association of Ontario.
  • Ability to work individually and as part of a team, to prioritize tasks in order of urgency.
  • Excellent communication (verbal and written) and comprehension skills.
  • Developed creative problem-solving skills to look at the "big picture"
Additional Requirements Include
  • A negative vulnerable sector check completed within 6 months of start date.
  • Completion of a 2 Step TB Skin test within the last 12 months of start date.
