Job summary

Southbridge Care Homes is seeking a Registered Practical Nurse (RPN) for Garden City Manor in St Catharine, Ontario. This part-time position covers evenings and days, offering responsibility in medication administration, treatments, and resident assessments to support high-quality care.

You will participate in multidisciplinary meetings, uphold College of Nurses of Ontario standards, and mentor nursing staff while maintaining infection control practices and a compassionate approach to seniors.

Qualifications

• Certificate of competence from the College of Nurses of Ontario.
• Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
• Current First Aid and BLS certification.
• Comprehensive knowledge of nursing and infection control practices.
• Effective communication and ability to work independently and collaboratively are essential.

Responsibilities

• Ensure resident care follows established nursing standards, physician orders, and policies; observe, report and record vital signs, symptoms and conditions; initiate remedial health measures as needed.
• Assess residents and develop, implement, review and evaluate care plans; monitor health, note changes, and assess need for referrals; implement physician orders.
• Organize and direct nursing and personal care staff; orient/train new personnel; participate in in-service education; monitor performance and conduct reviews.
• Plan, supervise and evaluate the work of nursing and personal care staff; coordinate with Director of Care.

Role

The RPN plays a pivotal role in enriching the lives of residents by supporting the LTC Home in the administration of medications, treatments, and assessments. This position actively participates in all multidisciplinary meetings to ensure excellence in resident care and uphold the quality of nursing practice in accordance with the following standards:

• College of Nurses of Ontario Standards
• Long-Term Care Standards
• Southbridge Care Homes Standards

Key Responsibilities

• Ensure resident care is provided with established nursing standards and principles, physicians’ orders, and administrative policies, as well as observe, report and record vital signs, symptoms and conditions of residents, and initiate remedial health measures where appropriate. Assure observance of required techniques in nursing care.
• Assess residents and develop, implement, review and evaluate resident care plans. Monitor residents’ health, note changes in condition, and assess need for referral to doctors; process and implement physician orders.
• Organize and direct functions and activities related to the provision of care to residents including restorative care and palliative care as required. Interpret and implement established policies, procedures, and regulations.
• Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in‑service education. Monitor performance and conduct performance appraisal and refer problems to the Director of Care.

Knowledge And Skills Required

• Appropriate current certificate of competence from the College of Nurses of Ontario required.
• Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
• Current First Aid and BLS Certificate.
• Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.
• Effective human relation skills and demonstrated commitment to working with seniors.
• Effective communication skills and ability to work independently and collaboratively are essential.

Additional Requirements

• A negative vulnerable sector check completed within 6 months of start date.
• Completion of a 2‑Step TB Skin test within the last 12 months of start date.
