Job summary

Southbridge Care Homes at Garden City Manor in St Catharine, Ontario, is seeking a Registered Practical Nurse (RPN) to support medication administration, treatments, and assessments.

You will participate in multidisciplinary meetings and ensure nursing practice meets College of Nurses of Ontario and Southbridge Care Homes standards, while collaborating with care staff to uphold high-quality resident care. Evening and night shifts are available.

Qualifications

• Certificate of Competence from the College of Nurses of Ontario.
• Registration or eligible for registration with the Registered Practical Nurses Association of Ontario.
• Current First Aid and BLS Certificate.
• Comprehensive knowledge of nursing and health care practices, including infection control.
• Effective human relation skills and commitment to working with seniors.
• Strong communication and ability to work independently and collaboratively.

Responsibilities

• Ensure resident care is provided to established nursing standards and follow physicians' orders and policies; observe and record vital signs and conditions.
• Assess residents and develop, implement, review and evaluate care plans; monitor health and refer to doctors when needed.
• Coordinate care provision including restorative and palliative care; implement policies and regulations.
• Plan, supervise and evaluate work of nursing staff; orient/train new personnel and conduct performance appraisals.

REGISTERED PRACTICAL NURSE

Salary Range: $33.30 per hour

Position: Registered Practical Nurse (RPN)

Evenings: 2pm - 10pm

Nights: 10pm - 6am

Reports to: Director of Care

The RPN plays a pivotal role in enriching the lives of residents by supporting the LTC Home in the administration of medications, treatments, and assessments. This position actively participates in all multidisciplinary meetings to ensure excellence in resident care and uphold the quality of nursing practice in accordance with:

• College of Nurses of Ontario Standards
• Long-Term Care Standards
• Southbridge Care Homes Standards

Key Responsibilities:

• Ensure resident care is provide with established nursing standards and principles, physicians' orders and administrative policies, as well as observe, report and record vital signs, symptoms and conditions of residents, and initiate remedial health measures where appropriate. Assure observance of required techniques in nursing care.
• Assess residents and develop, implement, review and evaluate resident care plans. Monitor residents' health, notes changes in condition and assess need for referral to doctors; processes and implement physician orders.
• Organize and direct functions and activities related to provision of care to residents including restorative care and palliative care as required. Interprets and implements established policies, procedures, and regulations.
• Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education. Monitor performance and conduct performance appraisal and refers problems to the Director of Care.

Knowledge and Skills Required:

• Appropriate current certificate of competence from the College of Nurses of Ontario required.
• Registration or eligible for registration with the Registered Practical Nurses Association of Ontario.
• Current First Aid and BLS Certificate
• Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.
• Effective human relation skills and demonstrated commitment to working with seniors.
• Effective communication skills and ability to work independently and collaboratively are essential

Additional Requirements include

• A negative vulnerable sector check completed within 6 months of start date.
• Completion of a 2 Step TB Skin test within the last 12 months of start date.
