Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents and guide Personal Support Workers on the unit. You will collaborate with the RN to implement policies and liaise with physicians and the broader health care team.

You will monitor resident status, communicate changes promptly, and participate in shift reports to keep the team informed. A current Ontario Nursing Certificate of Competence and active standing are required.

Qualifications

• Geriatric, rehabilitation or long-term care nursing background is required, including activation and personnel administration.
• Knowledge of relevant Ontario acts and standards (Nurses’ standards of practice, provincial acts).
• Current Certificate of Competence from the College of Nurses of Ontario and in good standing.

Responsibilities

• Collaborate with the RN to implement AgeCare policies and procedures on the unit.
• Liaise with physicians and other health care team members, including external partners.
• Notify physician of significant changes in resident status when RN is unavailable.
• Provide direction to Personal Support Workers on the care team.
• Participate in shift reports to inform the team of changes in resident status.
