Job summary

Scarborough Health Network (SHN) is seeking a Registered Practical Nurse to join our team at the Centenary Site. You will assess, plan, implement and evaluate nursing care for a diverse patient caseload across post-surgical, cardiac, stroke, and chronic conditions.

The role requires current RN/RO license with the College of Nurses of Ontario, BCLS CPR certification, and relevant assessments and rehabilitation coursework.

Qualifications

• Current certification with the College of Nurses of Ontario.
• CPR at Basic Cardiac Life Support level.
• Health Assessment course or equivalent experience.
• Rehabilitation courses from a recognized institution.
• Gerontology courses or equivalent experience.
• Current adult medical / surgical experience.
• Excellent communication, documentation, and interpersonal skills.
• Ability to work independently and in a multidisciplinary team.

Responsibilities

Job Description

Position Overview:Reporting to Manager of Continuing Care, the successful candidate will be responsible for assessing, planning implementation and evaluating nursing care and practice activities in a multidisciplinary environment. You will be providing care to a diverse patient caseload of post surgical, cardiac, stroke, musculoskeletal, progressive disorders, complex chronic illness, and dementia patients.

Requirements

• Current certification with the College of Nurses of Ontario
• Competent to perform CPR at a Basic Cardiac Life Support level
• Health Assessment course or equivalent experience
• Rehabilitation courses from a recognized institution
• Gerontology courses or equivalent experience
• Current adult medical / surgical experience
• Evidence of sound organizational and prioritization skills with the ability to work well in stressful situations
• Excellent communication, documentation, and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community
• Ability to work well independently and to exercise sound judgement
• Ability to perform the essential duties of the position
• Demonstrated initiative and the ability to function both independently and as a multidisciplinary team member in a fast paced demanding environment
• Demonstrates values consistent with SHN's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
