Job summary

Halton Region operates three non-profit long-term care homes, including Post Inn Village Long Term Care Facility in Oakville. This temporary nursing role involves front-line nursing care, resident communication, and a collaborative team approach to quality care.

As a valued member of the Social & Community Services team, you will ensure high-quality customer service, uphold ethical standards, and support a CARE-focused Model of Care while adhering to RN/ROP guidelines.

Qualifications

• Current registration with the College of Nurses of Ontario.
• Successful completion of an approved medication course for the RPN.
• Strong interpersonal and communication skills; collaboration and leadership.
• Ability to manage multiple demands with sound decision making and organization.
• Understanding of the Residents' Bill of Rights and LTC standards.

Responsibilities

• Provide nursing care per established standards.
• Contribute to assessment and documentation of residents' health status.
• Assist in planning and evaluating the Plan of Care.
• Provide direction and support to front line staff.
• Communicate effectively with residents, families, physicians, and staff.
• Administer medications and maintain drug security.
• Participate in Quality Improvement activities.
• Assist in orientation of new staff and volunteers.
• Act as resident advocate and coordinate with agencies as needed.

Job Summary

Participate as a member of the nursing and personal care team, providing front line staff with direction and support. This includes communicating with residents, staff, families and physicians while providing high quality customer service.

Duties & Responsibilities

• Provision of nursing care in accordance with established standards of nursing practice set by the College of Nurses of Ontario and Services for Seniors policies and procedures.
• Contributeto the assessment and documentation of each resident‘s health status on an ongoing basis.
• Assistin the development and implementation of the Plan of Care and evaluates its ongoing effectiveness.
• Providedirection and support to front line staff.
• Communicateeffectively with residents, staff, families, physicians and team members.
• Provideexcellent customer service at all times.
• Provideindividualized service and createa “homelike” atmosphere.
• Observereports and records symptoms and conditions of residents.
• Administermedication as necessary. Responsible for maintaining the security of drugs and narcotics according to applicable policies and legislation.
• Participatein Quality Improvement and Risk Management activities.
• Assistin the orientation of new staff, students and volunteers.
• Actas a resident advocate; communicates information about client /resident needs to internal and external agencies, staff, volunteers, families and students as required.
• Recommendchanges to nursing practice, care policies and procedures as the need arises.
• Contributeto physician rounds and facilitates Resident Care Conferences.
• Supportand promotethe “CARE” philosophy and Model of Care.
• Performother duties as assigned.

Skills & Qualifications

• Current registration with the College of Nurses of Ontario.

• Flexibility in work schedule including shifts and every other weekend is required.
• Must be able to perform all the necessary physical tasks associated with the care and assistance of residents.
