Candidates are invited to apply for the BSO RPN position to work at our Halton Hills home, a 130-bed long- term care facility, located in Georgetown, Ontario.

Reporting to Director of Care (or designate) you will be responsible for the provision of quality, resident-centered care based on the College of Nurses of Ontario and the Ministry of Health and Long-Term Care legislated requirements, Extendicare Values, policies and procedures, and best practice guidelines.

The Behavioural Supports Ontario Nurse is a member of the interdisciplinary team and works within the Behavioural Supports Ontario framework to support persons living with responsive behaviours associated with dementia, mental health or other complex neurological conditions.

Job Description:
  • Utilizes effective communication and interpersonal skills when interacting with residents and family members, as well as, internal and external care partners;
  • Delivers person-centered care which recognizes both the uniqueness of each resident and an awareness of one’s own contribution to the relationship, including personal attributes, values, and action;
  • Must be prepared to respond to emergency situations.
  • Provides behavioural support, guidance, and leadership to the interdisciplinary team.
  • Provides knowledge transfer of behaviour management strategies to build capacity of team members through one-to-one group sessions, role modeling, Prevention and Management of Responsive Behaviour Program, and behavioral huddles.
  • Promoting the residents’ integration to the environment.
  • Collaborates, supports and provides information to residents/clients and families regarding responsive behaviours and plan of care.
  • Completes responsive behavioural referrals received from the interdisciplinary team and provides recommendations in a timely manner.
  • Facilitates referrals to external resources and implements plan of care based on external consultant recommendations in collaboration with the interdisciplinary team.
  • Promotes resident/client centered approach, and more consistency of care.
  • Provides point of care education to provide effective interventions to residents/clients.
  • Uses assessment tools based on best practices to conduct initial and on-going assessments describing interventions with respect to the behaviors of the residents;
  • Documenting observational behaviors, recognizing that they have meaning, assessing and accounting for how the behaviors impact the care delivery process;
  • Assessing the meaning and identifying the behavioral risk using objective, systematic and holistic processes that take into account the P.I.E.C.E.S.;
  • Assists with the admission process of residents identified with responsive behaviours.
  • Provides leadership in implementing the Responsive Behaviour Program.
  • Completes applicable behavioural
  • Identifying caregiver strategies that are resident-centered age appropriate and maintain the resident’s dignity when responding to the behaviors and managing the associated risks; Ensures that there is a current plan of care in place for the resident with responsive behaviours that directs the staff in providing care and mitigating risk to the resident, other residents, staff members, and care providers;
  • Liaises with external partners to support resident-centered care;
  • Responds to resident or family member concerns and ensures appropriate action is taken within decision-making authority and/or brings to the attention of the Director of Care;
  • Understands and upholds all rules and requirements under applicable legislation (including but not limited to the Employment Standards Act, Human Rights Code, Ontario Labour Relations Act, the Ontario Health and Safety Act, Long Term Care Homes Act);
  • Works safely to reduce the risk of injury to self, other staff members and residents.
  • Performs other delegated duties as assigned.
What you bring
  • Possesses and maintains RPN certification with the College of Nurses of Ontario as well as other specialized qualifications to this position.
  • Demonstrates knowledge in dementia care, delirium, mental health issues and the impact that it has on the resident's quality of life, family members and care partners.
  • Must be available for flexible hours in order to meet the Behavioral needs of residents exhibiting responsive behaviors.
  • Current CPR and First Aid Certification.
  • Must have effective oral and written communication skills in English, a good understanding of resident/client-centered care.
  • Must be able to communicate appropriately with residents/clients, staff, visitors, volunteers and medical professionals.
  • Ability to approach highly sensitive issues with professionalism.
  • Proficiency in basic computer skills including the ability to create reports is a requirement.
  • Must be physically able to perform all delegated duties.
  • Flexible work hours and ability to cover all shifts. Hours may be determined by Director of Care or designate as per operational needs.

Schedule:

75 hours biweekly. Duties may be re-assigned as per operational needs of the residents.
