Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively with the interprofessional care team, the Registered Nurse/Registered Psychiatric Nurse provides treatment and care management services that are inclusive and flexible for individuals with mental illness and concurrent disorders. The role emphasizes recovery and rehabilitation, reducing barriers to care, and promoting a culturally-safe, trauma-informed environment. The nurse establishes a welcoming therapeutic environment and promotes client self-determination and independence.

Responsibilities
  1. Coordinates care for clients by collaborating with the interprofessional team, performing intake, conducting assessments, developing and adjusting treatment plans, administering medications, and monitoring clients' mental state and response to treatment.
  2. Plans and provides direct nursing care, including reviewing history, conducting assessments, developing care plans, and implementing treatment modalities such as ADLs, wound care, and medication administration.
  3. Works from a strengths-based model, using motivational interviewing and engagement techniques, and collaborates with clients and professionals to develop individualized care plans and access community services.
  4. Establishes and maintains therapeutic relationships, engages stakeholders, supports self-care, and educates clients and families about psychiatric conditions and management strategies.
  5. Provides treatment services including crisis intervention, counselling, and education, assessing situations, exploring emotional responses, and reviewing learning needs.
  6. Participates in case conferences with clients, families, and team members to coordinate care and advocate for clients' rights.
  7. Collaborates with the care team, including physicians, primary care providers, and other health professionals, to adjust care plans and address client needs.
  8. Monitors Extended Leave under the Mental Health Act, ensuring compliance and continuity of care.
  9. Assists clients in accessing services, including medical appointments and social programs.
  10. Documents assessments, care plans, interventions, and outcomes according to standards and procedures.
  11. Maintains a safe environment by adhering to safety practices and reporting risks.
  12. Identifies personal learning needs and engages in activities for professional development.
  13. Participates in research and quality improvement initiatives.
  14. May plan and deliver educational sessions and facilitate group interventions.
  15. May provide outreach nursing care in various settings, including home and community environments.
  16. Performs other duties as assigned.
Qualifications

Education, Training & Experience

Graduation from an approved School of Nursing or Psychiatric Nursing. Two (2) years recent related experience or an equivalent combination. May require completion of a substance use certificate and other specific courses.

Current registration with BCCNM. Valid BC Class 5 Driver's License and access to a personal vehicle.
