Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively with the interprofessional care team, the Registered Nurse/Registered Psychiatric Nurse provides treatment and care management services that are inclusive and flexible for individuals with mental illness and concurrent disorders (e.g., substance use, developmental disorders). The role focuses on meeting client needs, ensuring integration and continuity across the care continuum, and promoting recovery and rehabilitation. The nurse works within an interdisciplinary team-based model, fostering rapid engagement and delivering client and family-centered care with an emphasis on cultural safety, trauma-informed approaches, client self-determination, and independence.

Responsibilities
  1. Coordinates care by collaborating with the interprofessional team, conducting assessments, developing and adjusting treatment plans, administering medications, and monitoring client responses to ensure optimal resource use and care quality.
  2. Provides direct nursing care, including assessments, care planning, and implementing treatment modalities such as ADLs, wound care, and medication administration, in compliance with practice standards.
  3. Works from a strengths-based model, utilizing motivational interviewing and engagement techniques to develop individualized care plans and assist clients in accessing community services.
  4. Establishes and maintains therapeutic relationships with clients and their support networks, promoting self-care, informed decision-making, and quality of life improvements. Educates clients and families on managing psychiatric conditions and integrating illness into daily life.
  5. Delivers treatment services like crisis intervention, counseling, and education, assessing situations, exploring emotional responses, and reviewing learning needs.
  6. Participates in case conferences with clients, families, and team members to coordinate care, advocate for clients' rights, and address ongoing needs.
  7. Collaborates with healthcare professionals, including physicians, primary care providers, and educators, to adjust care plans and respond to changes in client conditions.
  8. Monitors extended leave under the Mental Health Act, ensuring compliance and continuity of care.
  9. Assists clients in accessing services, including medical appointments and social programs, and supports form processing and transportation needs.
  10. Documents assessments, care plans, interventions, and outcomes according to standards and procedures.
  11. Maintains a safe environment by adhering to safety protocols and reporting risks.
  12. Engages in ongoing professional development and participates in research and quality improvement activities.
  13. May deliver educational sessions and facilitate group interventions, including outreach nursing care for clients in various settings.
  14. Performs other related duties as required.
Qualifications

Education, Training & Experience

Graduation from an approved School of Nursing or Psychiatric Nursing. Two (2) years recent related experience or an equivalent combination of education, training, and experience. May require completion of a substance use certificate program and related courses.

Current registration as a Registered Nurse and/or Registered Psychiatric Nurse with BCCNM. Valid BC Class 5 Driver's License and access to a personal vehicle with appropriate insurance.

Skills and Abilities
  • Proficiency in mental health and substance use assessments and nursing care, including crisis intervention.
  • Knowledge of mental illness, substance use treatment, and psychopharmacology.
  • Strong counseling, group facilitation, and communication skills.
  • Understanding of relevant legislation, policies, and procedures.
  • Ability to promote person-centered, culturally safe, and trauma-informed care.
  • Effective organizational, collaborative, and research-informed decision-making skills.
  • Physical ability to perform duties and operate related equipment.
