Shannex Incorporated is seeking a Part-time Registered Nurse for the West Bedford Transitional Health team in Nova Scotia. You will support patient transitions from hospital to home, employing coordinated, evidence-based nursing practices and interprofessional collaboration.

Qualifications

  • Bachelor of Science in Nursing and current registration with the Provincial Association of Registered Nurses.
  • Proof of CPR/First Aid Certification is required.
  • Previous supervisory experience is beneficial.

Responsibilities

  • Assess, plan, implement, and evaluate nursing care.
  • Perform health assessments to maintain patient quality of life.
  • Manage patient medications per legislative requirements and MAR documentation.
  • Ensure care tasks are documented using the facility's technology and tools.
  • Collaborate with Continuing Care Assistants/PSWs to deliver timely care.
  • Develop, implement, and evaluate care plans in partnership with patients and families.
  • Support quality improvement, risk management, and adherence to policies.

About The Opportunity
  • Assesses, plans, implements, directs, and evaluates nursing care and functions, in accordance with the patient-directed approach to care through the established philosophy of service.
  • Performs initial and regular health assessments to maintain patient quality of life.
  • Assessing, planning, implementing and evaluating nursing care of all patients.
  • Manages patient medication in accordance with legislative requirements.
  • Follows the process in place for safekeeping of medications
  • Administers and records prescribed medications in accordance with facility policy.
  • Ensures all completed care tasks are documented appropriately using identified technology and tools.
  • Ensures Continuing Care Assistants/PSWs are completing care in a timely manner.
  • Ensures Continuing Care Assistants/ document all completed care, incidents, and errors accurately.
  • Applies dressings and treatments according to physician’s orders and facility policy.
  • Development, implementation and evaluation of care plans.
  • Observes, records and reports symptoms, reactions, treatments and changes in the patient’s condition.
  • Documents according to facility policy e.g Daily Patient Care Record, Medication Administration Record (MAR), patient progress notes, initiating medication incident reports, incident reports, and other facility specific documentation as required.
  • Supports continuous quality improvement, risk management and quality assurance activities.
  • Ensures organizational policies are adhered to by team.
  • Ensures patient safety by ensuring team members follow and apply all standard operational procedures, safe work practices and training provided.
  • Additional duties as required
About You
  • A Bachelor of Science in Nursing and are currently registered with the Provincial Association of Registered Nurses
  • Proof of valid CPR/First Aid Certification required
  • Successful candidates will be required to obtain a clear Criminal Record Check and a Vulnerable Sector Check
  • Demonstrated supervisory skills and ability to evaluate employee job performance
  • Specialized courses such as: Palliative Care and Venipuncture and Dementia: Understanding the Journey is considered an asset
  • Previous experience supporting admission and discharge of vulnerable populations with complex social situations; implementing care plans and identifying resources that will give the patient every opportunity to be successful in returning to home/community is considered an asset
