Shannex Incorporated in Nova Scotia is hiring a Licensed Practical Nurse for the West Bedford Transitional Health team to support hospital-to-home transitions with compassionate, patient-centered care.

The role involves interprofessional collaboration, medication administration, and leading Continuing Care Assistants to ensure safe, effective care plans while pursuing ongoing education in a supportive, safety-focused environment.

Qualifications

  • Graduate from an approved Practical Nursing Program.
  • Current registration with provincial Practical Nursing regulatory body.
  • Minimum two years nursing experience.
  • Proof of valid CPR & First Aid Certification.

Responsibilities

  • Performs initial and regular health assessments to maintain patient quality of life.
  • Assists with assessing, planning, implementing and evaluating nursing care.

Reporting to the Manager of Transitional Services, the LPN is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.

About The Opportunity
  • Performs initial and regular health assessments to maintain patient quality of life.
  • Assists with assessing, planning, implementing and evaluating nursing care of all patients.
  • Ensures compliance with Philosophy of Service and patient care plan to ensure optimal health and wellness for each patient.
  • Administers and records prescribed medications in accordance with facility policy.
  • Provides consultation regarding patient issues and standards of care to interdisciplinary team.
  • Ensures Continuing Care Assistants/PSWs are completing care tasks in a timely manner.
  • Ensures Continuing Care Assistants/ document all completed care, incidents, and errors accurately.
  • Assists patients with hygiene and daily living activities, such as bathing, dressing, oral care, skin care, meals, positioning in bed, and transferring to and from bed to chair.
  • Assists with patient’s special needs, e.g. eye care, colostomy, and catheter.
  • Applies dressings and treatments according to physician’s orders and facility policy.
  • Assists with the development, implementation and evaluation of care plans.
  • Documents according to facility policy e.g Daily Patient Care Record, Medication Administration Record (MAR), patient progress notes, initiating medication incident reports, incident reports, and other facility specific documentation as required.
  • Provides leadership and direction to team by fostering and promoting best practices.
  • Assists with the supervision and evaluation of job performance and behaviour of the Continuing Care Assistants.
  • Orientates and trains new Team Members on all departmental standards, procedures, and policies.
  • Assists in the coordination of patient care by adjusting assignments to meet care needs according to the number of and experience level of available staff.

About You
  • Graduate from an approved Practical Nursing Program
  • Approved Pharmacology and Physical Assessment course (if not completed during diploma)
  • Current registration with provincial Practical Nursing regulatory body
  • Minimum two years nursing experience
  • Demonstrated supervisory skills and ability to evaluate employee job performance.
  • Proof of valid CPR & First Aid Certification (required)
  • Successful candidates will be required to obtain a?clear Criminal Record Check, and?where applicable, a Vulnerable Sector Check, prior to appointment
  • Adheres to the?Standards of Nursing Practice, Code of Ethics, and Regulations?as defined by provincial legislation
  • Maintains current knowledge of literature and applied research for the delivery of health care
  • Maintains knowledge of current legislation and program requirements that govern
  • Specialized courses such as: Foot Care, Palliative Care, Venipuncture considered an asset
  • Additional course completion, such as Dementia: Understanding the Journey and Back in Form Lift & Transfer Program considered to be an asset
  • Bilingualism in French/English considered an asset
  • Previous experience supporting admission and discharge of vulnerable populations with complex social situations; implementing care plans and identifying resources that will give the patient every opportunity to be successful in returning to home/community.
