Qualifications

• Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM).
• Current certification in Basic Cardiac Life Support (BCLS).

Responsibilities

• Assesses patients and collects information to develop and update care plans.
• Collaborates with patients and families to implement and modify care plans.
• Coordinates nursing care and referrals with the interprofessional team.
• Educates patients and families about health goals and self-care after discharge.
• Documents assessments, interventions and outcomes in accordance with Fraser Health procedures.
• Participates in quality improvement and professional development activities.
